- Child Abuse Background Check
- PA State Background Check
- PDE 6004 Arrest/Conviction Report and Certification Form
- FBI Fingerprinting OR Signed Form of PA Residency for 10 Consecutive Years
- Mandated Reporter Training Certificate
- Tuberculous Test
- Signed Volunteer Agreement
- Completed and Signed Volunteer Agreement
