Voluntary Student Accident Plan Enrollment Link

There will be a lot of information coming home with your child during the first week of school. The following forms will need to be completed, signed, and returned to the school office.

Active Duty: Returned only if a student has a parent/guardian in active duty status
Bus Drop-off Authorization (K4, KDG, 1st ONLY): Lists authorized adults to pick up the students at the bus stop
Emergency Card: Provides the school with important medical information
Emergency Operation Plan and Student Release Authorization: Lists authorized adults designated to pick up your child in case of emergency
Media Release Form: Only returned if parent/guardian do NOT want their child’s image published
Peanut Allergy Letter: Parent Verification of severe nut allergies present
