Athletic Physical Information

DECEMBER 23 ATHLETIC PHYSICALS WILL BE LIVE on Arbiter Registration (FamilyID) on October 1.

 

Student-athlete physical examinations must be completed on or after June 1, 2022.

 

All Arbiter Registration (FamilyID) Athletic Physical submissions are due by NOVEMBER 10, 2022.

 

**PLEASE ONLY UPLOAD THE SECTION 7 CIPPE FORM -- ALL OTHER FORMS ARE HANDLED BY ANSWERING THE QUESTIONS ONLINE. ONLY UPLOAD SECTION 7 (UNLESS ATHLETE WAS INJURED LAST SEASON, THEN SEE BELOW). THE NECESSARY FORMS CAN BE FOUND BELOW, NO OTHER FORMS ARE NEEDED. **

 

If athlete was injured last season and this is his/her first sport/season of the school year, you must upload SECTION 7 along with SECTION 8 and 9.

If athlete was injured last season and this is his/her second season of the year, SECTION 8 and 9 must be uploaded with SECTION 7 that should have already been uploaded in the first season of the year. 

 

The process in which learners will submit their PIAA Comprehensive Initial-Pre Participation Physical Examination (Athletic Physical Packet) and supporting information will continue with Arbiter Registration (FamilyID).  Arbiter Registration is a secure registration platform that provides learners and families with an easy, user-friendly way to register for all athletics.  When you register through Arbiter Registration, the system keeps track of your information in a profile.

All of the information that used to be tediously entered on the physical packet can now be entered online with the exception of Section 7 (the Medical Examination Form) - this will still be a hard copy that athletes must have completed by a physician.  Upon completion of the actual physical examination, you will be able to upload the form into your Arbiter Registration account.  You can download/print Section 7 to take to the physical examination by clicking the attachment below.

 

All late registrations will not be guaranteed the opportunity to practice on the first day of practice.  

 

Important to know when completing your physical information in FamilyID:

You must complete all information in Arbiter Registration, upload the PIAA CIPPE physical form -- section 7 – after being completed by the physician. You NEED to make sure that the physical form is signed and dated by a medical professional after June 1, 2022.  This form needs scanned or photographed and uploaded in FamilyID where it says Section 7 form. Please delete all other physicals you have previously uploaded before you upload your newest physical.
