What forms need to be turned in for each new school year for the nurse? Per Pennsylvania Department of Education and School Health
Elementary
Kindergarten- 1. Up to date immunizations from a physician office (provided no later than the 5th day of school), a signed exemption from immunizations form/card or have a documented medical plan to update immunizations from a physician office
2.Physical exam (a child’s well visit, completed within a year of the first day of school)
- Dental exam (completed within one year of the first day of school)
1st Grade: None Required
2nd Grade: None Required
3rd Grade: Dental Exam (completed within one year of the first day of school)
4th Grade: None Required
5th Grade: None Required
Middle School-
6th Grade: Physical Exam and Scoliosis exam(completed within one year of the first day of school)
7th Grade: Dental Exam (completed within one year of the first day of school)
Immunizations (Tdap and MCV)
8th Grade: None Required
High School-
9th Grade: None Required
10th Grade: None Required
11th Grade: Physical Exam (completed within one year of the first day of school)
12th Grade: Second MCV Immunization
What are the yearly health screenings completed for your child?
*These are State mandated
- Height and weight - all students
- Vision Screening - all students
- Hearing Screening - K, 1, 2, 3, 7 and 11
- Physical Exam -K, 6, 11, or on original entry (Private physician encouraged)
- Dental Exam - K, 3, 7, or on original entry (Private dentist encouraged)
- Scoliosis Screening – grades 6 and 7 (Private physician encouraged)
When to keep your child home from school:
- Temperature 100.4 or greater – Please keep your child home until fever-free for 24 hours without the use of fever reducing medication.
- Vomiting or Diarrhea – Please keep your child home until 24 hours vomiting or diarrhea free
- Pink eye/eye infection – Please keep your child home until 24 hours of antibiotics or as directed by your physician
- Lice – Please keep your child home and call the school nurse for further directions. Click on the link for more information regarding lice treatment.

Chickenpox (Varicella)
Doctor diagnosed: students excluded from school until all vesicles are scabbed/crusted.
Coronavirus (COVID-19)

Fifth's Disease
Students should be excluded if they are febrile. No exclusion necessary for rash.
Influenza or Seasonal Flu
Students should be fever free for 24 hours without the aid of a fever reducing medication. Please consider cough status as well. Persistent cough interferes with your child's ability to participate in the learning process. Cough should be under control before returning to school.

Measles symptoms usually appear in 7 to 14 days, but could be as long as 21 days, after exposure to the virus. Common measles symptoms include:
- High fever (may spike to more than 104° F)
- Cough
- Runny nose
- Red, watery eyes
- Rash
Mononucleosis
Students may return when fever free and able to resume normal activities per physician recommendation.
Strep Throat/Scarlet Fever (Group A Streptococcal (GAS) Disease)
Students excluded from school until 24 hours after treatment is started.
